STAR CD Plan - Application Form

Please follow the following instructions to download and print the Application form.

  1. Click the desired printer format from the link under "Application Forms" below (e.g. HP Deskjet Format).
  2. Save the file in a known location.
  3. Use the appropriate method in your sytem to print the file.
Generally, you should be able to print in DOS environment using the DOS command

copy file_name printer_name

(where file_name is the name of the saved file and printer_name is LPT1 or LPT2, etc. e.g. copy boi_form.dj5 lpt1)

You are required to fill in and submit, duly signed, either

W-8 Form Postscript Format (Certificate of Foreign Status) - Required for accounts opened with foreign addresses:
or
W-9 Form Postscript Format (Request for Taxpayer Identification Number and Certification) - Required for accounts of USA Residents.
